Subject: IB/hfi1: Document phys port state bits not used in IB
Patch-mainline: v4.14-rc1
Git-commit: 55774d09b7a1f0d2bfec3320ed71d30fe05466a1
References: bsc#1060463 FATE#323043

A couple bits are used by OPA for link physical state that are not present
as part of InfiniBand. Add a short blurb what those states mean and removed
an unused state.
