Subject: [PATCH] nvme-fc: Sync nvme-fc header to FC-NVME-2
Git-commit: f16583614222d015968541f2e50447c67c277f74
Patch-mainline: v5.5-rc1
References: bsc#1169045

Sync the header to FC-NVME-2 r1.06 (T11-2019-00210-v001).

Includes some minor mods where pre-release field names changed
by the time the spec was released.
