Subject: [PATCH] scsi: bsg: Decouple from scsi_cmd_ioctl()

Decouple bsg from scsi_cmd_ioctl().  This requires a small amount of code
duplication, but will allow moving all SCSI ioctl handling into SCSI
midlayer.
